We size our belts based on pants size. For example, our Heritage Leather Men's Belt - Medium fits from a 37.5" - 43.5" waist (approx. 33.5" to 39.5" pants size). The middle hole (there are 7 holes) would be about a 40.5" waist (approx. a 36.5" pants size).
